Background:

Obstructive jaundice in a 48-year-old Saudi male.
Initial investigations suggested malignancy (cholangiocarcinoma, lymphoma, or metastasis) due to lymphadenopathy.
Preoperative diagnosis remained uncertain.

Observation:

Endoscopic retrograde cholangiopancreatography revealed distal common bile duct (CBD) stricture.
Computed tomography scan showed lymphadenopathy along the CBD and porta hepatis.
Laparotomy confirmed enlarged lymph nodes in the porta hepatis and along the stomach's lesser curvature.

Findings:

Castleman disease of the hyaline vascular type was confirmed by pathological examination of lymph nodes.
The patient underwent a triple bypass procedure to alleviate biliary obstruction.
