Diagnosing primary pericardial mesothelioma is challenging, often mistaken for metastatic cancer. This rare tumor has a poor prognosis, highlighting diagnostic difficulties.

Area of Science:

  • Oncology
  • Pathology

Background:

  • Pericardial effusion and mediastinal lymphadenopathy can indicate malignancy.
  • Metastatic lung cancer is a common cause of pericardial involvement.

Observation:

  • A patient initially diagnosed with papillary adenocarcinoma metastatic to the pericardium was treated without success.
  • Re-examination of pericardial biopsy revealed calretinin positivity, indicating a different diagnosis.

Findings:

  • The final diagnosis was primary pericardial mesothelioma of the epithelioid type.
  • The tumor was initially misdiagnosed due to its rarity and presentation.
  • Chemotherapy for suspected metastatic lung cancer was ineffective.

Implications:

  • This case underscores the diagnostic challenges associated with primary pericardial mesothelioma.
