Summary

Adequate preoperative hydration and lactulose effectively prevent postoperative renal dysfunction in patients with acute obstructive jaundice (AOJ). This management strategy is crucial for safeguarding kidney function, even in high-risk elderly individuals.

Area of Science:

  • Gastroenterology
  • Nephrology
  • Surgical Management

Background:

  • Acute obstructive jaundice (AOJ) can lead to reduced plasma volume and endotoxemia.
  • Optimal preoperative management for AOJ remains debated.
  • Preoperative hydration and lactulose are investigated for preventing renal dysfunction.

Purpose of the Study:

  • To demonstrate the necessity of preoperative hydration and lactulose.
  • To prevent postoperative renal dysfunction in patients with AOJ.

Main Methods:

  • 55 patients with AOJ (benign/malignant strictures) received oral lactulose and IV cefazoline preoperatively.
  • Fluid balance was maintained, and high-risk patients underwent echocardiography.
  • Renal function assessed via creatinine clearance (ClCr) changes post-surgery.

Related Experiment Videos

Main Results:

  • No patients developed postoperative renal failure.
  • Preoperative and postoperative ClCr values showed no significant difference (P=0.489).
  • One patient with pre-existing renal impairment showed improved ClCr post-operation.

Conclusions:

  • Adequate preoperative hydration and endotoxin inactivation are vital for preventing acute renal failure in AOJ.
  • This regimen effectively prevents renal complications, even in high-risk elderly patients.
  • The study supports the use of this protocol for AOJ management.
