Background:

Retroperitoneal fatty tumors are rare, with lipomas, liposarcomas, and extra-renal angiomyolipomas (AML) as key differentials.
Accurate diagnosis is crucial for appropriate management and surgical planning.

Observation:

A 48-year-old female presented with a 6.2 kg giant mixed-type liposarcoma in the left perinephric space.
The tumor invaded the renal cortex and parenchyma, posing a diagnostic challenge against benign conditions like AML.

Findings:

Surgical extirpation with left radical nephrectomy was performed.
Histology confirmed a liposarcoma with both myxoid and well-differentiated elements.
The patient remained disease-free at 6-month follow-up.
