Summary

Surgical repair of thoracoabdominal aneurysms has a high mortality rate, especially for symptomatic or ruptured cases. Early detection and elective repair significantly improve patient survival outcomes.

Area of Science:

  • Cardiovascular Surgery
  • Vascular Surgery
  • Aortic Aneurysm Research

Background:

  • Thoracoabdominal aneurysms (TAAs) are complex vascular conditions requiring surgical intervention.
  • Surgical repair of TAAs historically carries significant risks and mortality.

Purpose of the Study:

  • To analyze the outcomes of surgical repair for thoracoabdominal aneurysms.
  • To identify factors influencing mortality and morbidity in TAA repair.

Main Methods:

  • Retrospective review of 129 patients undergoing surgical repair of thoracoabdominal aneurysms between May 1966 and June 1991.
  • Analysis of perioperative mortality, complications, and long-term outcomes.

Main Results:

  • Overall 30-day mortality was 35%. Elective repair had 15% mortality versus 63% for symptomatic/ruptured aneurysms (p<0.001).

Related Experiment Videos

  • Spinal cord ischemia occurred in 21% of survivors, with higher rates in extensive aortic replacement (Crawford groups I, II).
  • Complications included myocardial infarction (11%), respiratory failure (36%), and renal failure (27%).
  • Conclusions:

    • Early detection and elective surgical repair of thoracoabdominal aneurysms are crucial for improving early survival.
    • Patient outcomes are significantly worse for symptomatic or ruptured aneurysms compared to elective cases.
