Summary

Hyperparathyroidism significantly enhances aminolevulinic acid (ALA)-induced fluorescence in parathyroid glands. This fluorescence difference could enable safer, minimally invasive surgery for parathyroid diseases.

Area of Science:

  • Biomedical Engineering
  • Surgical Oncology
  • Endocrinology

Background:

  • Intraoperative parathyroid gland localization is difficult, particularly in minimally invasive surgery.
  • Aminolevulinic acid (ALA) fluorescence diagnosis has shown potential for identifying normal parathyroid glands.
  • This study investigates fluorescence differences between hyperplastic and normal parathyroid glands for clinical application.

Purpose of the Study:

  • To evaluate the fluorescence characteristics of hyperplastic versus normal parathyroid glands using ALA.
  • To determine if ALA-induced fluorescence can differentiate parathyroid glands in hyperparathyroidism.
  • To establish a precondition for clinical application of ALA fluorescence in parathyroid surgery.

Main Methods:

  • Polycystic kidney disease (PKD) rats (hyperplastic glands) and Wistar rats (normal glands) were administered ALA.

Related Experiment Videos

  • Fluorescence of parathyroid and thyroid glands was assessed under blue light (d-light system).
  • Spectrometry measured fluorescence intensity; serum parathyroid hormone levels were determined by ELISA.
  • Main Results:

    • PKD rats exhibited stronger red fluorescence in parathyroid glands compared to Wistar rats.
    • Spectrometry showed 10x higher fluorescence in PKD rat parathyroid glands vs. thyroid, versus 3.2:1 in Wistar rats.
    • Parathyroid glands in PKD rats were over twice as fluorescent as in Wistar rats; ELISA confirmed hyperparathyroidism.

    Conclusions:

    • Hyperparathyroidism intensifies ALA-induced fluorescence in parathyroid glands.
    • This enhanced fluorescence may support unilateral, minimally invasive approaches in parathyroid surgery.
    • The technique could improve surgeon's ability to manage parathyroid diseases safely and efficiently.
