Asymptomatic microbleeds in moyamoya disease: T2*-weighted gradient-echo magnetic resonance imaging study

Background:

  • Moyamoya disease (MMD) is a rare cerebrovascular disorder characterized by progressive stenosis of the internal carotid arteries.
  • Asymptomatic microbleeds (MBs) are increasingly recognized as potential indicators of cerebrovascular disease activity.

Observation:

  • This study retrospectively analyzed 63 MMD patients and 34 healthy volunteers using 3-tesla (3T) and 1.5-tesla (1.5T) MR imaging.
  • Gradient-echo T2*-weighted sequences were employed to detect MBs.

Findings:

  • Asymptomatic MBs were detected in 44% of MMD patients using 3T MR imaging, compared to 28% with 1.5T MR imaging.
  • The incidence of MBs in MMD patients (44%) was significantly higher than in healthy controls (5.8%) when assessed with 3T MR imaging.
  • MBs were found in both ischemic (44%) and hemorrhagic (43%) MMD subtypes.

Implications:

  • 3-tesla MR imaging significantly improves the detection rate of asymptomatic MBs in MMD patients.
  • Increased prevalence of MBs in MMD suggests their role as a potential biomarker for disease activity and progression.
