Summary

Surgical reconstruction techniques for complex middle cerebral artery aneurysms offer effective solutions when clipping or coiling is not feasible. These methods provide good to excellent outcomes for patients with challenging vascular conditions.

Background:

  • Direct clipping and endovascular coiling are standard treatments for cerebral aneurysms.
  • Unclippable and uncoilable middle cerebral artery (MCA) aneurysms present significant treatment challenges.

Observation:

  • A retrospective review of eight operations in seven patients over nine years was conducted.
  • Patients had complex MCA bifurcation aneurysms unsuitable for standard treatments.

Findings:

  • Various reconstructive techniques were employed, including saphenous vein graft bypass, radial artery grafts, and direct branch reimplantation.
  • No mortality occurred, with six patients achieving excellent outcomes (Glasgow Outcome Scale 5) and one a good outcome (GOS 4).
