Summary

Pulmonary involvement in ulcerative colitis (UC) may be detected early using technetium-99m diethylene triamine penta acetic acid (Tc-99m DTPA) aerosol scintigraphy. This test reveals increased alveolar epithelial permeability, an extraintestinal manifestation in UC patients, even without respiratory symptoms.

Background:

  • Pulmonary dysfunction is observed in ulcerative colitis (UC) patients, but its pathogenesis is not fully understood.
  • This study investigates subclinical alveolar epithelial damage in UC patients using Tc-99m DTPA aerosol scintigraphy.

Purpose of the Study:

  • To assess alveolar epithelial damage in UC patients without respiratory symptoms.
  • To determine if Tc-99m DTPA aerosol scintigraphy can detect early pulmonary changes in UC.

Main Methods:

  • 32 active UC patients, 10 inactive UC patients, and 31 healthy controls underwent Tc-99m DTPA aerosol scintigraphy.
  • Alveolar epithelial permeability was measured and correlated with disease activity, localization, and duration.

  • Significantly higher alveolar epithelial permeability was found in both active and inactive UC patients compared to controls (P < 0.001).
  • No correlation was observed between Tc-99m DTPA test results and disease activity, localization, or duration.

  • Latent pulmonary involvement, indicated by increased alveolar epithelial permeability, may be present in both active and inactive UC.
  • Tc-99m DTPA clearance tests can identify early pulmonary epithelial damage preceding clinical symptoms.
  • Increased alveolar epithelial permeability is an extraintestinal manifestation of UC, independent of colitis activity.
