Background:

  • A 22-year-old male with a history of generalized tonic-clonic seizures presented with spinocerebellar ataxia.
  • Electrophysiological studies indicated a demyelinating motor-sensory neuropathy.

Observation:

  • Serum vitamin E levels were found to be low.
  • Sural nerve biopsy showed loss of large myelinated fibers with signs of remyelination.

Findings:

  • Vitamin E supplementation resulted in clinical improvement and electrophysiological recovery of sensory conduction and evoked potentials.
  • Motor nerve conduction demonstrated only partial recovery following supplementation.

Implications:

  • This case suggests that vitamin E deficiency can cause demyelinating neuropathy with variable recovery.
