Summary

Cyclosporin and mycophenolate mofetil (MMF) combined with PUVA therapy show promise for treating extensive diffuse morphoea, a condition difficult to manage with existing treatments. This combination approach led to considerable improvement in a patient with this rare skin disorder.

Area of Science:

  • Dermatology
  • Immunosuppression

Background:

  • Cyclosporin and mycophenolate mofetil (MMF) are established immunosuppressants used in organ transplantation.
  • Cyclosporin is also utilized for dermatological conditions like psoriasis and eczema.
  • Extensive diffuse morphoea presents a significant therapeutic challenge, with limited success for conventional treatments like PUVA and UVA, especially in advanced stages.

Observation:

  • The patient had extensive diffuse morphoea, a condition notoriously difficult to treat.
  • Previous treatments with PUVA and UVA, along with other immunomodulating drugs, offered limited benefit, particularly in established disease.

Findings:

  • A patient with extensive diffuse morphoea was treated sequentially with cyclosporin and PUVA, followed by MMF and PUVA.
  • This combined therapeutic approach resulted in considerable improvement of the patient's morphoea.

Related Experiment Videos

Implications:

  • Combination therapy, specifically cyclosporin/MMF with PUVA, may represent a viable treatment strategy for extensive diffuse morphoea.
  • Further research into combination immunosuppressive and phototherapy regimens is warranted for refractory morphoea.
  • This case highlights a potential new avenue for managing severe and widespread morphoea.
