Area of Science:

  • Psychiatry
  • General Practice
  • Epidemiology

Background:

  • Common mental disorders (CMD) are prevalent in primary care settings.
  • Accurate case-finding is crucial for timely and effective treatment.
  • Existing screening tools may have limitations in diverse primary care populations.

Purpose of the Study:

  • To validate the external validity of a new case-finding instrument, the Common Mental Disorders Questionnaire (CMDQ).
  • To assess the diagnostic accuracy of the CMDQ compared to the Schedules for Clinical Assessment in Neuropsychiatry (SCAN) as a gold standard.
  • To evaluate the CMDQ's utility as a diagnostic aid in primary care.

Main Methods:

  • A cross-sectional, two-phase study involving 1785 adult patients across 28 general practices.

Related Experiment Videos

  • Patients completed screening questionnaires for somatisation, anxiety, depression, and alcohol abuse.
  • A subsample underwent the SCAN interview, serving as the gold standard for validation.
  • Main Results:

    • The CMDQ demonstrated high sensitivity and specificity for diagnosing depressive, anxiety, and alcohol use disorders.
    • For somatisation disorder, SCL-SOM showed 83% sensitivity and 56% specificity, while Whiteley-7 showed 75% sensitivity and 52% specificity.
    • Overall, the CMDQ exhibited higher diagnostic accuracy than general practitioners across evaluated diagnoses.

    Conclusions:

    • The Common Mental Disorders Questionnaire (CMDQ) possesses excellent external validity.
    • The CMDQ is a valuable diagnostic aid for identifying common mental disorders in primary care.
    • Implementation of the CMDQ can improve case-finding and subsequent patient management.
