Early familial non-medullary thyroid cancer: differences with late-onset and sporadic forms.

Subclinical hypothyroidism (SH) and even early thyroid dysfunction in euthyroid patients show impaired cardiac systolic and diastolic function. Pulsed wave tissue Doppler imaging (PWTDI) detects these cardiac changes before TSH levels rise.

Background:

  • Subclinical hypothyroidism (SH) is associated with impaired diastolic function at rest and during exercise.
  • Systolic dysfunction in SH has primarily been assessed during exercise, with limited data on resting function.
  • Cardiac dysfunction may precede the rise in thyroid-stimulating hormone (TSH) in individuals at risk for SH.

Purpose of the Study:

  • To evaluate resting systolic function in patients with SH.
  • To determine if cardiac dysfunction occurs in euthyroid individuals at high risk for developing SH.
  • To investigate the relationship between cardiac function and thyroid hormone levels.

Main Methods:

  • Study included 32 patients with Hashimoto's thyroiditis (22 with elevated TSH - Group A, 10 with normal TSH - Group B) and 13 healthy controls (Group C).

  • Pulsed wave tissue Doppler imaging (PWTDI) was used to assess global and regional left ventricular function.
  • Correlations between PWTDI parameters and serum TSH, free T3, and free T4 concentrations were analyzed.
  • Main Results:

    • PWTDI revealed significant impairment in systolic ejection, diastolic relaxation, and ventricular filling compliance in both SH groups (A and B) compared to controls (C).
    • These impairments were observed even in patients with normal TSH levels (Group B).
    • Significant correlations were found between PWTDI indices and serum TSH, FT3, and FT4 levels.

    Conclusions:

    • Pulsed wave tissue Doppler imaging (PWTDI) is a sensitive method for detecting both diastolic and systolic abnormalities in subclinical hypothyroidism.
    • Cardiac dysfunction, including systolic and diastolic abnormalities, can be detected in euthyroid individuals at high risk for thyroid failure.
    • Findings suggest a continuum of thyroid failure in autoimmune thyroiditis, with cardiac dysfunction preceding TSH elevation.
