A 60-year-old woman experienced syncope due to ventricular tachycardia (VT). Surgical treatment involving the Dor operation and endocardial cryoablation successfully resolved the VT, preventing recurrence.
Area of Science:
- Cardiology
- Cardiac Surgery
- Electrophysiology
Background:
- Ventricular tachycardia (VT) can cause syncope, often linked to coronary artery disease.
- Severe coronary stenosis and left ventricular aneurysm are significant risk factors for VT.
Observation:
- A 60-year-old woman presented with syncope secondary to sustained VT.
- Coronary arteriography showed critical stenosis in the right and left descending arteries and a left akinetic aneurysm.
Findings:
- The patient underwent a Dor operation combined with endocardial cryoablation for VT.
- Postoperative recovery was uneventful, with no clinical recurrence of VT observed.
Implications:
- This case highlights the efficacy of surgical VT ablation in patients with complex coronary artery disease and left ventricular dysfunction.
- Combined surgical and ablation strategies can offer a definitive solution for refractory VT.
- Successful management prevents recurrent syncope and improves patient outcomes.
