Summary

Pentoxifylline may improve heart failure symptoms and cardiac function, but current trials are too small for definitive conclusions. Larger studies are needed to confirm its effectiveness in heart failure management.

Background:

Recent trials suggest pentoxifylline benefits heart failure patients by reducing inflammation and improving cardiac dysfunction markers.
Routine use of pentoxifylline in heart failure management remains uncertain.

Purpose of the Study:

To evaluate the effectiveness of pentoxifylline in treating heart failure.

Main Methods:

Systematic review of randomized controlled trials (RCTs) published up to November 2004.
Searched MEDLINE and Cochrane Controlled Trials Register for relevant RCTs.
Assessed New York Heart Association (NYHA) functional class, left ventricular ejection fraction (LVEF), hospitalization frequency, and all-cause mortality.

Main Results:

Four studies with 144 participants met inclusion criteria; meta-analysis was not performed due to heterogeneity.

Pentoxifylline showed significant symptom (NYHA class) and cardiac function (LVEF) improvement in 3 of 4 studies.

A trend towards reduced mortality was observed in all studies, but not statistically significant; hospitalization effects were not assessed.

Conclusions:

Pentoxifylline may offer benefits for heart failure symptoms, ejection fraction, and mortality.
Current evidence is limited by small trial sizes, necessitating large-scale, placebo-controlled studies in diverse heart failure populations.
