    Background:

    • Glucocorticosteroids are commonly used for immunosuppression in myasthenia gravis.
    • Controlled studies on their efficacy are limited.

    Purpose of the Study:

    • To assess the efficacy of glucocorticosteroids or adrenocorticotrophic hormone (ACTH) in autoimmune myasthenia gravis.

    Main Methods:

    • Searched Cochrane, MEDLINE, and EMBASE databases up to June 2004.
    • Included quasi-randomized or randomized controlled trials of all ages and severity.
    • Primary outcome: muscle strength improvement; Secondary outcomes: remission, worsening episodes, antibody titres.

    Main Results:

    • Adrenocorticotrophic hormone (ACTH) showed no advantage over placebo for ocular myasthenia gravis.
    • Prednisone demonstrated significant short-term improvement compared to placebo in generalized myasthenia gravis.
    • No significant differences in efficacy were found between corticosteroids and azathioprine or intravenous immunoglobulin.

    Conclusions:

    • Randomized controlled trials suggest short-term benefits of corticosteroids for myasthenia gravis.
    • These findings align with observational studies and expert opinions.
    • Current evidence does not support superiority over azathioprine or intravenous immunoglobulin.
