Clinical decision support systems for neonatal care.

K Tan, P R F Dear, S J Newell

The Cochrane Database of Systematic Reviews
    |April 23, 2005

    Summary

Limited studies show clinical decision support systems (CDSS) have minimal impact on neonatal mortality and morbidity. Further randomized trials are needed to evaluate CDSS effectiveness in neonatal medicine.

    Area of Science:

    • Neonatal Medicine
    • Medical Informatics
    • Clinical Research

    Background:

    • Clinical decision support systems (CDSS) are computer-based tools aiding patient care decisions.
    • Existing research on CDSS effectiveness is primarily in adult medicine.
    • There is a lack of systematic reviews on CDSS in neonatal care.

    Purpose of the Study:

    • To evaluate the impact of CDSS on neonatal mortality and morbidity.
    • To assess the effect of CDSS on the performance of physicians treating newborns.

    Main Methods:

    • Systematic review using Cochrane Neonatal Review Group methods.
    • Searched multiple databases including CENTRAL, MEDLINE, EMBASE, CINAHL, and AMED.
    • Included randomized or quasi-randomized controlled trials comparing CDSS with no CDSS in neonatal care.

    Main Results:

    • Two studies on computer-aided prescribing showed no significant short-term effects, but one reduced drug calculation errors and time.
    • One study on computer-aided physiological monitoring found no significant impact on mortality or intraventricular hemorrhage.
    • Long-term neurodevelopmental outcomes could not be analyzed due to limited data.

    Conclusions:

    • Current randomized trial data on CDSS in neonatal care is very limited.
    • Further rigorous evaluation of CDSS in neonatal medicine is warranted.
    • The potential benefits and risks of CDSS in this population require more investigation.
