Warm ischemic time during laparoscopic live donor nephrectomy: effects on graft function.

Summary

Warm ischemia time (WIT) during laparoscopic live donor nephrectomy (LDN) did not impact short-term or long-term kidney graft outcomes. However, prolonged WIT may be linked to a higher rate of acute rejection episodes post-transplant.

Area of Science:

  • Nephrology
  • Transplant Surgery
  • Minimally Invasive Surgery

Background:

  • Laparoscopic live donor nephrectomy (LDN) is a standard alternative to open surgery.
  • The impact of prolonged warm ischemia time (WIT) on kidney graft outcomes after LDN requires further investigation.

Purpose of the Study:

  • To analyze the effects of WIT duration on short-term and long-term graft outcomes following LDN.

Main Methods:

  • Retrospective analysis of LDN procedures.
  • Assessment of WIT effects on delayed graft function, serum creatinine levels, acute rejection rates, and graft survival.
  • Comparison of outcomes based on WIT duration: <=3 min vs >3 min, and <5 min, 5-10 min, >10 min.

Main Results:

Related Experiment Videos

  • WIT ranged from 1 to 15 minutes, influenced by the learning curve and technical challenges.
  • Prolonged WIT did not affect early graft function or serum creatinine decline within the first 3 months.
  • An increased rate of acute rejection may be associated with longer WIT, though changes in Delta creatinine were unaffected.
  • Conclusions:

    • Warm ischemia time (WIT) during laparoscopic live donor nephrectomy (LDN) does not influence short-term or long-term kidney graft outcomes.
    • The study suggests WIT may correlate with acute rejection rates, warranting further research.
