Background:

  • Giant cell bone tumors (GCBT) are typically benign but can exhibit aggressive local behavior.
  • Multicentric presentation of GCBT is rare, posing treatment challenges.

Observation:

  • A 28-year-old woman presented with seven giant cell bone tumors across multiple locations including the spine (Th3), humeri, tibiae, femur, and hip bone.
  • Treatment involved local resection and alcohol adjuvant therapy, with radiotherapy avoided due to patient age and tumor distribution.

Findings:

  • Initial treatment resulted in no recurrence in the long bones and hip bone.
  • A relapse occurred at Th3 within six months, requiring reoperation.
  • A new tumor was detected in the distal right tibia during follow-up.

Implications:

  • Multicentric giant cell bone tumors necessitate vigilant monitoring due to potential for late recurrence and new tumor development.
  • Local resection with adjuvant alcohol therapy may be effective for localized GCBT, but multicentricity warrants tailored, long-term surveillance strategies.
  • The findings highlight the unpredictable behavior of multicentric GCBT and the importance of ongoing follow-up for early detection of relapses or new lesions.
