Summary

Erythromycin effectively treated idiopathic gastroparesis in two patients when standard prokinetic agents failed. This offers a new therapeutic option for refractory gastroparesis cases.

Background:

  • Gastroparesis, a condition impairing gastric emptying, is often treated with prokinetic agents.
  • Standard treatments include metoclopramide and domperidone, but efficacy is variable.

Observation:

  • Two patients with idiopathic gastroparesis had failed to respond to metoclopramide and/or domperidone.
  • Erythromycin was administered as a prokinetic agent in these refractory cases.

Findings:

  • Successful treatment of idiopathic gastroparesis was achieved using erythromycin in both reported cases.
  • Erythromycin demonstrated prokinetic effects, improving gastric motility.

Implications:

  • Erythromycin represents a viable alternative prokinetic therapy for gastroparesis when conventional agents are ineffective.
  • Further investigation into erythromycin's role in managing gastroparesis is warranted.
