Contact allergy to methyldibromo glutaronitrile (MDBGN) is a concern. A 0.2% patch test concentration is best for MDBGN, but no safe leave-on product concentration was found.

Background:

  • Contact allergy to methyldibromo glutaronitrile (MDBGN), often with phenoxyethanol (PE), rose in Europe during the 1990s.
  • The European Commission banned MDBGN in leave-on products in 2003 due to high use concentrations and unknown safe levels.

Purpose of the Study:

  • To determine the maximum non-eliciting concentration of MDBGN in leave-on products for sensitized patients.
  • To identify the optimal patch test concentration for diagnosing MDBGN allergy.

Main Methods:

  • Repeated Open Application Test (ROAT) with 3 MDBGN/PE concentrations (50, 100, 250 p.p.m.) in 39 sensitized patients.
  • Patch testing with 4 MDBGN concentrations (0.1%, 0.2%, 0.3%, 0.5%) in a subset of 24 patients.

Main Results:

  • 62% of patients showed a positive ROAT. 13 reacted to the lowest concentration (50 p.p.m.), developing dermatitis within days.
  • A 0.2% MDBGN patch test concentration achieved 94% sensitivity, but specificity decreased significantly with higher concentrations (0.5%).
  • No safe, microbicidal concentration for leave-on products was identified.

Conclusions:

  • 0.2% MDBGN is likely the most effective patch test concentration for diagnosing contact allergy.
  • 50 p.p.m. MDBGN in leave-on products can trigger contact dermatitis in sensitized individuals.
  • Identifying a non-eliciting concentration for leave-on products remains challenging.
