Summary

Microscopic colitis, specifically lymphocytic colitis, can affect elderly women, presenting with persistent watery diarrhea. Cholestyramine effectively treated these two reported cases.

Area of Science:

  • Gastroenterology
  • Pathology

Background:

  • Microscopic colitis encompasses lymphocytic colitis and collagenous colitis, diagnosed via biopsy.
  • Both present with chronic watery diarrhea but differ in microscopic features and clinical duration.
  • Lymphocytic colitis typically has shorter diarrhea duration and less female predominance than collagenous colitis.

Observation:

  • Two elderly women presented with two months of watery diarrhea and weight loss.
  • Initial investigations and treatments for their symptoms were unsuccessful.
  • Biopsies confirmed lymphocytic colitis in both patients.

Findings:

  • Lymphocytic colitis is characterized by increased intraepithelial lymphocytes in the colonic mucosa.
  • The patients' diarrhea significantly improved after treatment with cholestyramine.

Implications:

  • This case series suggests cholestyramine as a potential treatment for lymphocytic colitis in elderly women.
  • Highlights the importance of considering microscopic colitis in elderly patients with unexplained chronic diarrhea.
  • Further research may elucidate the efficacy of cholestyramine in a broader patient population.
