This case report details an exceptional anterior carpometacarpal dislocation in an 18-year-old male. Surgical reduction and K-wire fixation resulted in an excellent outcome one year post-injury.

Area of Science:

  • Orthopedic surgery
  • Traumatology
  • Hand surgery

Background:

  • Anterior carpometacarpal (CMC) dislocations are rare hand injuries.
  • Pure anterior dislocations involving all five CMC joints are exceptionally uncommon.

Observation:

  • An 18-year-old male student presented with a closed left hand injury after a motorcycle fall.
  • Examination revealed significant swelling and impaired mobility without neurovascular compromise.
  • Radiographs confirmed a pure anterior dislocation of all five carpometacarpal joints.

Findings:

  • Emergency surgical intervention was performed using combined anterior and posterior approaches.
  • Reduction of the dislocation was achieved and stabilized with Kirschner wires (K-wires).
  • An antebrachiopalmar cast was applied for six weeks post-operatively.

Implications:

  • This case highlights a successful surgical management strategy for a rare traumatic hand injury.
  • Prompt surgical reduction and fixation can lead to excellent functional recovery in anterior CMC dislocations.
  • Further research into optimal fixation methods for these rare injuries is warranted.
