Summary

Sleeping on your back (supine position) worsens central sleep apnoeas (CSAs) in patients with Cheyne-Stokes respiration (CSR). This position increases the frequency of breathing pauses during sleep, particularly in lighter sleep stages.

Background:

  • Cheyne-Stokes respiration (CSR) is often associated with cardiovascular diseases.
  • Central sleep apnoeas (CSAs) are common in patients with CSR.
  • The impact of sleep position on CSAs in CSR is not well understood.

Purpose of the Study:

  • To investigate the influence of body position and sleep stages on central sleep apnoeas.
  • To determine if sleep position affects the severity of CSAs in patients with CSR.

Main Methods:

  • Nocturnal polysomnography was performed on 20 patients with cardiovascular diseases and CSR.
  • A body position sensor was used to track patient positioning during sleep.
  • Central apnoea-hypopnoea index (CAHI) was calculated for different sleep positions and stages.

  • The mean CAHI was significantly higher in the supine position (41±13) compared to nonsupine positions (26±12).
  • CAHI was highest in sleep stages 1 and 2, and lowest in slow-wave sleep and REM sleep.

Conclusions:

  • Supine body position exacerbates central sleep apnoeas in patients with Cheyne-Stokes respiration.
  • Sleep position is a significant factor influencing CSA severity in this patient population.
  • Avoiding the supine position may help manage CSAs in patients with CSR.
