Exercise therapy offers slight benefits for chronic low back pain, improving pain and function. For acute low back pain, exercise is as effective as other treatments, but evidence for subacute cases is inconsistent.

- In patient-seeking-care populations, improvements were more pronounced (13.3 points for pain, 6.9 points for function). Evidence for subacute LBP was inconsistent, while acute LBP showed exercise therapy to be equally effective as other conservative treatments.
Conclusions:
- Exercise therapy shows modest efficacy in reducing pain and enhancing function for chronic low back pain, particularly in healthcare-seeking populations.
- For subacute LBP, graded-activity programs show some promise for improving absenteeism, though evidence for other exercise types remains unclear.
- In acute LBP, exercise therapy provides comparable outcomes to no treatment or other conservative interventions, but study limitations necessitate cautious interpretation.
