Background:

  • Fever of unknown origin (FUO) and skeletal pain are challenging diagnostic problems in children.
  • Bartonella species are increasingly recognized as causes of human infections, including osteomyelitis.

Observation:

  • Two pediatric cases presented with prolonged FUO and significant skeletal pain.
  • Initial conventional radiologic studies (plain films, scintigraphy, CT) failed to identify the cause of bone abnormalities.

Findings:

  • Magnetic resonance imaging (MRI) demonstrated multiple enhancing bone marrow lesions in both children.
  • Serological testing confirmed Bartonella infection as the etiology of the multifocal bone disease.
