Summary

Background:

Myasthenia gravis can lead to acute respiratory failure.
Patients may present with symptoms mimicking acute myocardial infarction.

Observation:

A male patient with myasthenia gravis was hospitalized for acute respiratory failure.
He exhibited electrocardiographic and enzymatic changes suggestive of myocardial infarction.

Findings:

Coronary angiography revealed no severe coronary obstruction.
Ventriculography showed significant left ventricular systolic dysfunction, characteristic of Takotsubo syndrome (transient ventricular dysfunction).
Complete recovery of electrocardiographic alterations and ventricular function was confirmed by echocardiography.
