Summary

Investigating uterine anomalies via hysteroscopy after two pregnancy losses is as effective as waiting for three or more. This approach may be justified earlier for recurrent pregnancy loss patients.

Area of Science:

  • Reproductive Medicine
  • Gynecologic Surgery

Background:

  • Recurrent pregnancy loss (RPL) evaluation traditionally starts after three or more miscarriages.
  • A growing trend suggests earlier investigation after two miscarriages, but supporting data is limited.

Purpose of the Study:

  • To compare the frequency of uterine anomalies in women undergoing hysteroscopy for RPL after two versus three or more miscarriages.

Main Methods:

  • Retrospective analysis of uterine anomalies identified via hysteroscopy.
  • Study included patients referred for RPL at an academic medical center.

Main Results:

  • Hysteroscopy was performed on 165 women with RPL (67 after two, 98 after three or more losses).
  • The detection rate of uterine anomalies was similar: 32% after two losses versus 28% after three or more losses.

Conclusions:

  • Hysteroscopy may be a justified diagnostic tool after only two spontaneous pregnancy losses.
  • Earlier hysteroscopic evaluation for uterine anomalies in RPL cases warrants consideration.
