Summary

This case study presents aggressive thoracic spine diskitis with unusual imaging features. The condition resolved spontaneously, suggesting a possible link to Reflex Sympathetic Dystrophy (RSD) in the axial skeleton.

Area of Science:

  • Radiology
  • Spine Surgery
  • Neurology

Background:

  • Diskitis, an inflammation of the intervertebral disc space, can present with varied imaging characteristics.
  • Aggressive and destructive features on MRI typically suggest infectious or neoplastic processes.
  • Idiopathic cases of diskitis pose diagnostic challenges.

Observation:

  • A patient presented with thoracic spine discitis showing aggressive features on MRI.
  • Two biopsies were unremarkable, yielding no definitive cause.
  • Symptoms began after sneezing and included paraspinous enhancement.

Findings:

  • The condition resolved clinically and radiologically within four months without antibiotic treatment.
  • The presentation shares similarities with Reflex Sympathetic Dystrophy (RSD) in extremities.

Related Experiment Videos

  • This may represent the first imaging evidence of axial skeletal RSD.
  • Implications:

    • This case expands the understanding of potential causes and presentations of spinal inflammatory conditions.
    • It highlights the possibility of neuro-inflammatory processes like RSD affecting the axial skeleton.
    • Further research is needed to confirm RSD as a cause of spinal discitis-like presentations.
