Summary

Isolated monoparesis is a rare stroke symptom. This study found it often results from small artery disease or hemorrhage, challenging previous assumptions about lacunar infarcts.

  • Ischemic strokes accounted for 76.5% and hemorrhagic strokes for 23.5%.
  • Small artery disease (39.2%) and cardioembolism (15.7%) were leading causes of ischemic stroke; thalamic-capsular hemorrhages were common.

Conclusions:

  • Isolated monoparesis is an uncommon stroke presentation.
  • Small artery disease and small hemorrhages are frequent causes of isolated monoparesis.
