Summary

Electrocortical stimulation revealed language cortex in the right hemisphere of five of six patients with bilateral language. These findings suggest accessory language zones in the right hemisphere, warranting further research.

Background:

  • The language cortex is primarily localized to the dominant left hemisphere.
  • Language representation in the right hemisphere, especially in cases of bilateral language, is not well understood.

Purpose of the Study:

  • To investigate language representation in the right hemisphere using electrocortical stimulation (ECS).
  • To map language areas in patients with bilateral language and a right subdural electrode array (SEA).

Main Methods:

  • Retrospective review of six patients with bilateral language and right SEA implantation.
  • Language lateralization determined by preoperative intracarotid amobarbital procedure.
  • Anatomical localization verified by intraoperative photographs and MRI scans.
