Background:

  • Treatment-resistant psychogenic stiff neck and severe depression present complex clinical challenges.
  • Conventional treatments including antidepressants, neuroleptics, and antiepileptics were ineffective.

Observation:

  • A 48-year-old woman with comorbid psychogenic stiff neck and depression received sertraline and risperidone.
  • Significant improvement in depressive symptoms was noted within weeks.
  • Psychogenic stiff neck resolved after 6 months of risperidone monotherapy.

Findings:

  • Combination therapy with sertraline and risperidone showed efficacy in managing severe depression.
  • Risperidone monotherapy demonstrated effectiveness in resolving psychogenic stiff neck.

Implications:

  • Risperidone may offer a therapeutic option for neurological motor disorders co-occurring with mood disorders.
  • This suggests a potential role for atypical antipsychotics in managing complex neuropsychiatric conditions.
