Stylalgia diagnosis is complex, often involving temporomandibular joint disorders or Eagle syndrome. A rare cause, basicranium malformation, can dislocate the styloid process, leading to stylalgia.

Area of Science:

  • Neurology
  • Otolaryngology
  • Anatomy

Background:

  • Stylalgia presents a broad differential diagnosis, including temporomandibular joint disorders, neoplasms, and Eagle syndrome.
  • Basicranium malformation is a rare etiological factor for stylalgia, leading to styloid process dislocation.

Observation:

  • This case report details a diagnostic challenge and surgical approach for stylalgia.
  • The stylalgia was attributed to a dislocated styloid process secondary to basicranium malformation.

Findings:

  • Dislocation of the styloid process reduced the maxillo-vertebropharyngeal space.
  • This led to nerve fiber irritation and chronic stylopharyngeal muscle contraction.

Implications:

  • Basicranium malformation should be considered in stylalgia cases with normally dimensioned styloid processes.
