Summary

Common variable immunodeficiency (CVID) patients with fewer switched memory B cells show lower IgG levels and poorer vaccine responses. These patients also exhibit higher rates of autoimmune and granulomatous conditions.

Background:

  • Common variable immunodeficiency (CVID) is a heterogeneous disorder with unknown causes.
  • Subclassifying CVID patients may help understand disease mechanisms.
  • Memory B cell subsets, identified by CD27 expression, are potential CVID biomarkers.

Purpose of the Study:

  • To investigate the clinical significance of switched memory B cells in CVID patients.
  • To determine if specific B cell subsets correlate with disease severity and clinical manifestations.

Main Methods:

  • 53 CVID patients were categorized into two groups based on switched memory B cell percentages (CD27+IgM-IgD-).
  • Group I: <0.4% switched memory B cells; Group II: >0.4%.
  • Serum IgG levels and post-pneumococcal vaccine IgG antibody responses were measured. Rates of autoimmune and granulomatous diseases were recorded.

  • Group I patients had significantly lower median serum IgG levels (145 mg/dl) compared to Group II (329.5 mg/dl).
  • Group I showed a diminished response to pneumococcal vaccination (0.5 serotypes) versus Group II (3 serotypes).

Conclusions:

  • Decreased switched memory B cells in CVID are associated with reduced serum IgG and impaired vaccine response.
  • Lower switched memory B cell percentages correlate with increased risk of autoimmune and granulomatous complications in CVID.
  • Switched memory B cell counts may serve as a prognostic indicator for CVID patients.
