Background:

  • Herpes simplex virus (HSV) commonly causes tracheobronchitis and pneumonitis.
  • Endobronchial masses are rare manifestations of HSV infections, with only one prior report of HSV type II.

Observation:

  • A 68-year-old female with severe kyphoscoliosis developed blood-tinged endotracheal secretions after intubation for respiratory failure.
  • Fibreoptic bronchoscopy revealed an endobronchial mass in the right middle lobe.

Findings:

  • Cultures confirmed Herpes simplex virus type I (HSV-1) infection.
  • Biopsy specimens showed cytopathological changes consistent with HSV infection.

Implications:

  • This is the first reported case of HSV type I presenting as an endobronchial tumor.
