Summary

Wavefront LASIK for high myopia can increase spherical aberration, especially when pupil analysis diameter exceeds the laser optical zone. Consider larger analysis diameters to detect these changes accurately.

Background:

  • Laser-assisted in situ keratomileusis (LASIK) is a common refractive surgery.
  • Wavefront analysis helps to measure and correct higher-order aberrations (HOAs).
  • Spherical aberration and coma are key HOAs impacting visual quality.

Purpose of the Study:

  • To compare preoperative and postoperative higher-order aberrations (HOAs) after wavefront LASIK.
  • To evaluate the impact of varying pupil analysis diameters and myopic correction levels.
  • To determine optimal parameters for wavefront measurements in LASIK.

Main Methods:

  • Retrospective chart review of 21 eyes from 12 patients undergoing CustomCornea LASIK.
  • Wavefront analyses were performed preoperatively and 3 months postoperatively.

  • Pupil analysis diameters ranged from 5.5 to 7.0 mm for low, medium, and high myopic corrections.
  • Main Results:

    • No significant increase in postoperative coma or other HOAs was observed.
    • A significant increase in spherical aberration difference was noted for high myopic corrections (>6 D) at a 7.0 mm analysis diameter compared to a 6.5 mm zone.
    • Other analysis diameters and subgroups showed no significant differences.

    Conclusions:

    • The largest spherical aberration increase occurred when the analysis diameter (7.0 mm) was 0.5 mm larger than the optical zone (6.5 mm) in high myopes.
    • Using analysis diameters 0.5 mm larger than the laser optical zone is recommended to detect induced spherical aberrations.
