    Sevoflurane, a new halogen-containing anesthetic in Russia, provides smooth initial narcosis in adults. Its hypnotic effects necessitate fentanyl for adequate analgesia, with potential use in pediatric and ambulatory anesthesia.

    Background:

    • Sevoflurane is a novel halogen-containing anesthetic agent.
    • Its use in Russia for adult anesthesia was previously undocumented.

    Purpose of the Study:

    • To evaluate the initial clinical experience with sevoflurane for induction and maintenance of anesthesia in adult patients in Russia.
    • To compare the induction time of sevoflurane with propofol.

    Main Methods:

    • Sevoflurane was administered via mask for induction at 0.7%, increased to 2% after 3-5 breaths.
    • Induction time was assessed by achieving 40-45% on the index of Національний Електроенцефалограф (INEEG).
    • Comparison was made with propofol induction times.

    Main Results:

    • Induction with sevoflurane was smooth, with a mean time to INEEG 40-45% of 3.2 ± 0.5 min, comparable to propofol (3.3 ± 1.0 min).
    • Sevoflurane demonstrated a strong hypnotic effect but a weak analgesic effect in adults, requiring co-administration of fentanyl.
    • Minor, transient motor activity was observed in two patients.

    Conclusions:

    • Sevoflurane is effective for initial narcosis and anesthesia maintenance in adults, though supplemental analgesia is needed.
    • The agent is anticipated to be highly valuable for pediatric anesthesia, ambulatory settings, and same-day surgeries.
    • Further use in adult anesthesia induction and maintenance is supported by this initial experience.
