    • To comprehensively assess bronchial patency, lung volumes, capacities, and pulmonary gas exchange in patients with severe COPD (FEV1 < 50% predicted).

    Summary:

    • 36 severe COPD patients underwent pulmonary function testing.
    • Findings revealed impaired bronchial patency (reduced FEV1, FVC, PEF, MEF, increased Raw, Rin, Rex) and altered lung volumes (increased RV, TGV, TLC; decreased VC, FVC).
    • 83.3% of patients displayed pulmonary gas exchange dysfunction, with abnormal PaO2, PaCO2, and DLCO-SS.

    Impact:

    • This study highlights the significant and widespread physiological derangements in severe COPD.
    • The findings underscore the need for targeted therapeutic strategies addressing both airflow obstruction and gas exchange deficits.
