    Background:

    • Cardiac Syndrome X (CSX) is a condition characterized by angina-like chest pain with normal coronary angiograms.
    • Diastolic dysfunction, an impairment of the heart's ability to relax and fill, is increasingly recognized in various cardiovascular conditions.
    • Tissue Doppler imaging (TDI) is a valuable non-invasive tool for assessing myocardial function, particularly diastolic properties.

    Purpose of the Study:

    • To evaluate left ventricular (LV) diastolic function in patients with CSX using impulse tissue Doppler study.
    • To identify specific TDI parameters indicative of segmental diastolic dysfunction in CSX.
    • To explore the relationship between diastolic dysfunction, exercise capacity, and metabolic markers in CSX patients.

    Main Methods:

    • Impulse tissue Doppler imaging (TDI) was performed on 44 CSX patients and 46 healthy controls.
    • Maximal peak velocities (s, e, a), the e/a ratio, and segmental isovolumic relaxation time (IVRT) were measured in 18 LV segments.
    • Criteria for disturbed segmental diastolic function were defined as e/a ratio <1.0 and IVRT >85 ms.

    Main Results:

    • Abnormal LV filling (E/A (min) <1.0) was observed in 40.9% of CSX patients, all with segmental diastolic dysfunction.
    • Even among patients with normal overall LV filling (E/A (min) >1.0), 69.23% showed segmental diastolic dysfunction.
    • Patients with and without diastolic dysfunction differed in exercise tolerance, nocturnal blood pressure fall, and levels of glucose and LDL cholesterol.

    Conclusions:

    • Segmental diastolic dysfunction is prevalent in CSX patients, even with normal overall LV filling.
    • TDI parameters like e/a ratio and IVRT are sensitive indicators of diastolic impairment in CSX.
    • Diastolic dysfunction in CSX is associated with impaired exercise capacity and adverse metabolic profiles, suggesting a broader pathophysiological impact.
