Summary

Hippocampal malformations in patients with long-term epilepsy do not always progress to hippocampal sclerosis. Histological examination of two cases revealed developmental abnormalities without the characteristic neuronal loss of sclerosis.

Background:

  • Hippocampal malformations are implicated in the pathogenesis of hippocampal sclerosis, a frequent cause of intractable partial epilepsy.
  • This study investigates the progression of hippocampal malformations to hippocampal sclerosis in patients with chronic epilepsy.

Observation:

  • Two patients with decades of recurrent convulsive seizures and documented hippocampal and cortical developmental abnormalities were studied.
  • Histopathological examination included immunohistochemistry for dynorphin and neuropeptide Y.

Findings:

  • Neither patient exhibited neuronal loss in the hippocampi, a hallmark of hippocampal sclerosis.
  • Abnormal hippocampal convolutions (CA1 subfield and subiculum) and cortical malformations (polymicrogyria, heterotopia) were observed.

    • Hippocampal malformations may not inevitably progress to hippocampal sclerosis, even with long-standing epilepsy.

    • Further research is needed to elucidate the diverse pathways of epilepsy development.
