    Background:

    • Allopurinol is a common medication for gout and hyperuricemia.
    • Severe hypersensitivity reactions (HSRs) to allopurinol are known but potentially underreported.

    Purpose of the Study:

    • To investigate the frequency and characteristics of severe allopurinol hypersensitivity reactions.
    • To identify risk factors and clinical patterns associated with allopurinol-induced HSRs.

    Main Methods:

    • Retrospective study of patients with severe allopurinol HSRs over a five-year period.
    • Analysis of patient demographics, comorbidities, concomitant medications, clinical manifestations, and outcomes.

    Main Results:

    • Twenty patients experienced severe allopurinol HSRs.
    • Preexisting renal impairment and concurrent thiazide diuretic use were significant risk factors.
    • Cutaneous reactions ranged from maculopapular eruptions to toxic epidermal necrolysis.
    • Hepatic (40%) and renal (45%) involvement were common, usually reversible.
    • Mortality rate was 15% (3/20 patients).

    Conclusions:

    • Allopurinol hypersensitivity reactions are a serious concern with potentially underestimated frequency.
    • Careful patient selection and dose modification, especially in renal impairment, are crucial.
    • Increased awareness and monitoring are warranted for patients on allopurinol.
