Major depressive disorder (MDD) patients show reduced myo-inositol levels in the prefrontal cortex, suggesting potential glial alterations. This finding may offer new insights into the neurobiology of depression.

Background:

  • Major depressive disorder (MDD) is associated with prefrontal cortex glial changes.
  • Previous proton magnetic resonance spectroscopy (1H-MRS) studies on myo-inositol in MDD had potential confounds.
  • This study investigated glial marker myo-inositol levels in the prefrontal/anterior cingulate cortex in MDD.

Observation:

  • Thirteen nonmedicated MDD patients and 13 controls were studied using 3.0 T 1H-MRS.
  • Spectra were acquired from the prefrontal/anterior cingulate cortex.
  • Metabolite data were adjusted for tissue composition.

Findings:

  • MDD patients exhibited significantly lower myo-inositol/creatine ratios compared to controls.
  • The reduction in myo-inositol/creatine ratio was statistically significant (p=.016).

Implications:

  • Reduced myo-inositol in the prefrontal/anterior cingulate cortex may indicate glial loss or altered glial metabolism in MDD.
  • In vivo studies of glial markers are needed to further understand MDD pathophysiology.
  • These findings contribute to the neurobiological understanding of major depressive disorder.
