Critical aspects in the use of orthodontic palatal implants

The concept that osseointegrated implants might provide absolute anchorage has led to the introduction of customized orthodontic implants, such as the Orthosystem (Institut Straumann, Waldenburg, Switzerland). As this new technique evolves, especially with palatal implants, it is evident that many factors influence an implant's ease of use and clinical effectiveness. In particular, successful placement and subsequent usage depend on control of the profile drill's position, inclination, and speed. This article examines the critical factors at each stage of the clinical protocol and proposes a standardized approach for palatal implants. Particular emphasis is placed on a combined process of cephalometric and working model planning. The resultant implant prescription is then relayed to the placement stage by using a surgical stent with a locating guide plane, as introduced here.
