Scabietic nodules: a dermatopathologic and immunofluorescent study.

Summary

Mite parts are frequently found in scabietic nodules, suggesting they cause persistent inflammation. Vasculitis is uncommon, potentially appearing late in nodule development, and immunoreactant deposits may be secondary to inflammation rather than a specific immune response.

Area of Science:

  • Dermatology
  • Parasitology
  • Immunology

Background:

  • Scabies is a parasitic infestation caused by Sarcoptes scabiei mites.
  • Nodular lesions are a recognized clinical manifestation of scabies.
  • The pathogenesis of scabietic nodules, including the role of mite parts and immune responses, requires further elucidation.

Purpose of the Study:

  • To investigate the presence of mite parts in scabietic nodules.
  • To evaluate the occurrence of vasculitis in these lesions.
  • To assess the significance of immunoreactant deposition at the epidermodermal junction using direct immunofluorescence.

Main Methods:

  • Histopathological examination of serial sections from 27 scabietic scrotal papules/nodules.
  • Direct immunofluorescence (DIF) on 13 scabietic nodules to detect immunoreactant deposition.

Related Experiment Videos

  • Correlation of histopathological findings with clinical presentation and biopsy timing.
  • Main Results:

    • Mite parts were identified in 22% of scabietic nodules.
    • Vasculitis with fibrinoid degeneration was observed in only 3 out of 27 cases (11%).
    • Direct immunofluorescence showed immunoreactants at the epidermodermal junction in 31% of cases, with low intensity and limited immunoglobulin classes, suggesting a non-specific inflammatory process.

    Conclusions:

    • The presence of mite parts in scabietic nodules supports their role in persistent inflammation.
    • Vasculitis may be a late-developing feature in scabietic nodules.
    • Immunoreactant deposition at the epidermodermal junction in scabies is likely secondary to inflammation rather than a specific immunologic reaction.
