Postmenopausal hormone replacement therapy (HRT) does not appear to affect intraocular pressure (IOP). However, a history of ischemic heart disease may increase the risk of elevated IOP.

  • The impact of HRT on IOP remains unclear.

Purpose of the Study:

  • To investigate the association between HRT and intraocular pressure (IOP) in postmenopausal women.
  • To evaluate the effect of lifetime estrogen and progesterone exposure on IOP.

Main Methods:

  • Cross-sectional controlled study of 214 women (107 HRT users, 107 controls) aged 60-80.
  • Assessed IOP, cup-to-disc (C/D) ratios, and glaucoma history.
  • Collected data on HRT use, hormone exposure, and cardiovascular risk factors.

  • No significant differences in mean IOP, C/D ratios, or prevalence of increased IOP/glaucoma between HRT users and controls.
  • Ischemic heart disease history was the only significant risk factor for increased IOP (OR=4.63, P=0.003).
  • Lifetime estrogen/progesterone exposure did not significantly affect IOP risk.

Conclusions:

  • HRT and lifetime hormone exposure do not appear to influence IOP or the risk of increased IOP.
  • Ischemic heart disease may be a risk factor for elevated IOP in postmenopausal women.
