    Background:

    • Aneurysms of the infrapopliteal arteries are rare vascular conditions.
    • Systemic diseases, particularly collagen vascular diseases, can be associated with arterial aneurysms.
    • Posterior tibial artery aneurysms are an uncommon presentation within infrapopliteal aneurysms.

    Observation:

    • A 37-year-old man presented with calf pain and swelling.
    • Bilateral posterior tibial artery aneurysms were diagnosed using duplex scan and arteriography.
    • The patient was found to have a lupus-like syndrome, indicating a collagen vascular disease.

    Findings:

    • This case adds to the limited literature on infrapopliteal artery aneurysms.
    • Four of the ten previously reported cases of infrapopliteal artery aneurysms were associated with systemic diseases.
    • The current case demonstrates a link between posterior tibial artery aneurysms and a lupus-like syndrome.

    Implications:

    • Early diagnosis of infrapopliteal artery aneurysms is essential for effective management.
    • The association with collagen vascular diseases suggests a potential underlying systemic etiology.
    • A review of clinical manifestations and a treatment plan for these rare lesions are presented.
