Summary

Extended prostate biopsy (more than 10 cores) increases prostate cancer detection rates. However, it does not appear to increase the detection of clinically insignificant prostate cancer.

Background:

  • Extended prostate biopsy schemas are proposed to reduce false-negative cancer diagnoses.
  • Concerns exist regarding the potential for increased detection of clinically insignificant prostate cancer with extended schemas.

Purpose of the Study:

  • To evaluate the impact of extended prostate biopsy patterns on overall cancer detection.
  • To assess if extended biopsy patterns lead to a higher detection rate of clinically insignificant prostate cancer.

Main Methods:

  • A retrospective review of 740 patients undergoing prostate biopsy over 5 years.
  • Extended biopsy defined as >10 cores; clinically insignificant cancer defined by tumor size, Gleason score, and organ confinement.
  • Adjusted regression models analyzed the association between extended biopsy and cancer detection.
