Summary

This study compared healing and recurrence in chronic leg ulcers treated with skin grafting. Traumatic ulcers healed best, while vasculitic ulcers had high recurrence rates despite pain relief.

Area of Science:

  • Vascular Surgery
  • Dermatology
  • Wound Healing

Background:

  • Chronic leg ulcers affect a significant patient population, often requiring complex management.
  • Standard treatment involves surgical excision, skin grafting, and addressing underlying venous insufficiency.

Purpose of the Study:

  • To compare healing and recurrence rates of chronic leg ulcers based on their underlying aetiology.
  • To evaluate the effectiveness of surgical intervention (excision and skin grafting) in different ulcer types.

Main Methods:

  • A cohort of 385 patients with 406 chronic leg ulcers underwent standard treatment.
  • Treatment included ulcer excision, meshed split-skin grafting, and correction of superficial venous insufficiency.
  • Healing and recurrence rates were tracked over one year post-treatment.

Related Experiment Videos

Main Results:

  • An overall healing rate of 64% was observed after one year.
  • Traumatic ulcers demonstrated the highest healing rates (86%), while arterial ulcers had the lowest (20%).
  • Recurrence rates varied by aetiology: 14% for venous/ischaemic, 8% for traumatic, and a high 59% for vasculitic ulcers.

Conclusions:

  • Surgical treatment with skin grafting yields variable healing outcomes depending on leg ulcer aetiology.
  • Traumatic ulcers respond best to this treatment, whereas arterial ulcers show poor healing potential.
  • High recurrence rates in vasculitic ulcers necessitate further investigation, though grafting provides pain relief.
