Summary

This study presents a surgical technique to repair leaking filtering blebs by excising scarred tissue and using fresh conjunctiva to cover the filtration area. The method successfully restored filtration in five glaucoma patients.

Background:

Filtering blebs are used to treat glaucoma by creating a drainage pathway for aqueous humor.
Chronically thinned filtering blebs can develop leaks, compromising intraocular pressure control.
Limbus-based leaks in filtering blebs require effective surgical revision.

Purpose of the Study:

To describe a surgical technique for revising chronically-thinned filtering blebs with limbal leaks.
To evaluate the efficacy of this technique in repairing bleb leaks and restoring filtration.

Main Methods:

Surgical excision of scarred cystic conjunctiva and Tenon's fascia around the leaking bleb.
Mobilization of uninvolved conjunctiva and Tenon's fascia using a superior fornix relaxing incision.
Suturing mobilized tissue over the filtration area to create a fresh seal.

Main Results:

The technique was successfully applied in five cases of leaking filtering blebs.
The procedure provided fresh tissue to repair the bleb leak.
Adequate filtration was restored post-operatively in all treated cases.

Conclusions:

This surgical revision technique is effective for managing chronically-thinned, leaking filtering blebs.
