Area of Science:

  • Cardiology
  • Biomedical Engineering
  • Regenerative Medicine

Background:

  • Pathologic cardiac hypertrophy and dilation, termed reverse remodeling, are key therapeutic targets for dilated cardiomyopathies.
  • Pharmacologic therapies offer limited regression of cardiac remodeling.
  • Cardiovascular devices show promise for more effective reverse remodeling.

Purpose of the Study:

  • To review the role and efficacy of cardiovascular devices in promoting reverse remodeling.
  • To compare device-associated reverse remodeling with pharmacologic approaches.
  • To highlight the potential of passive cardiac support devices.

Main Methods:

  • Review of existing literature on cardiovascular devices for dilated cardiomyopathies.
  • Analysis of data from studies utilizing left ventricular assist devices.

Related Experiment Videos

  • Examination of clinical efficacy of multisite ventricular pacing.
  • Evaluation of passive cardiac support strategies.
  • Main Results:

    • Left ventricular assist devices induce dramatic reverse remodeling in advanced heart failure.
    • Multisite ventricular pacing reduces cardiac dilation and hypertrophy, improving symptoms and function.
    • Passive cardiac support devices present a promising strategy for preventing or reversing remodeling.

    Conclusions:

    • Cardiovascular devices offer a more rapid and reliable method for reverse remodeling compared to pharmacologic treatments.
    • Left ventricular assist devices and multisite ventricular pacing are effective interventions.
    • Passive cardiac support devices represent a novel and promising therapeutic avenue for dilated cardiomyopathies.
